The question of bacteriostatic water vs sterile water comes up every time someone opens a lyophilised vial for the first time. Both are sterile, both are “water for injection”, and both dissolve a freeze-dried peptide or hormone in seconds. The difference is a single excipient, benzyl alcohol at 0.9%, and that excipient decides whether the vial can be used once or entered repeatedly over four weeks.
Whichever way round you search it, sterile water vs bacteriostatic water or the reverse, the answer is the same. This article explains what each product is, what the labels actually say, when sterile water is the right choice and when it is the wrong one, and what the preservative does and does not protect against. What is bacteriostatic water?
Bacteriostatic Water for Injection, USP, is water prepared to the United States Pharmacopeia (USP) standard for injectable products, to which benzyl alcohol has been added at 0.9% (9 mg/mL) as a bacteriostatic preservative. It is labelled “for diluting or dissolving drugs for intravenous, intramuscular or subcutaneous injection” and “use only as a diluent or solvent”, and it is supplied in a multiple-dose container “from which repeated withdrawals may be made” [1][2]. In the UK it is sold in 30 mL vials for the same purpose. “Bac water” is simply the common abbreviation.
The preservative is the whole point. Each time a needle passes through the rubber stopper there is a small chance of carrying bacteria into the vial. Benzyl alcohol inhibits the growth of most of those organisms, so a vial that is handled aseptically can be used for multiple withdrawals rather than discarded after the first. What is sterile water for injection?
Sterile Water for Injection, USP, is the same pharmacopoeial-grade water without any additive. The B. Braun label describes it as “sterile, hypotonic, nonpyrogenic, and contains no bacteriostatic or antimicrobial agents”, and instructs that additives be introduced aseptically, mixed thoroughly and not stored [3]. It is a single-use product: once the container is opened, whatever is not used immediately is discarded.
Because it contains no solute, sterile water is strongly hypotonic. The label warns: “Do not inject until made approximately isotonic by addition of an appropriate solute, due to the possibility of hemolysis” [3]. Red cells placed in pure water swell and burst. This matters for injecting the water alone or in large intravenous volumes, and it is why neither product should ever be injected as-is.

Why multi dose bacteriostatic water is the standard for repeated withdrawals
The 28-day figure is not a marketing number. USP General Chapter <797>, the US standard for sterile compounding, states that a multiple-dose container “may be assigned a BUD [beyond-use date] of 28 days unless otherwise specified by manufacturer labeling”, regardless of the air quality in which it was opened. The US Centers for Disease Control and Prevention (CDC) says the same: once a multi-dose vial is opened it “should be dated and discarded within 28 days unless the manufacturer states another date” [4][5].
The contrast with a single-dose container is stark. Under USP <797>, a single-dose vial opened outside an ISO Class 5 environment (that is, anywhere other than a pharmacy clean-air hood) must be used within four hours, and even inside one it gets 12 hours [4]. A vial of unpreserved sterile water opened on a kitchen table falls into that four-hour category.
The practical consequence: if you reconstitute a peptide with sterile water and then draw from that vial for a fortnight, you have made an unpreserved multi-dose vial, and nothing in it is inhibiting whatever was introduced on day one. Reconstituting with bacteriostatic water is what makes the 28-day convention defensible for a vial that is entered repeatedly.
When sterile water is the right choice
Sterile water is not inferior; it is a different product for a different situation.
- Single-use reconstitution. If the entire vial is dissolved and administered at once, there is no need for a preservative. Several licensed products are supplied with sterile water or sodium chloride as the solvent for exactly this reason; the UK Pregnyl (hCG) leaflet says the solution “should be used immediately after reconstitution” [6].
- Neonates and, with caution, infants. Benzyl alcohol is contraindicated in neonates because of the “gasping syndrome”, a fatal metabolic acidosis first described by Gershanik and colleagues in 1982 in premature infants exposed to benzyl alcohol-preserved flush solutions [7]. The European Medicines Agency (EMA) links intravenous exposure of 100 to 200 mg/kg/day to the syndrome and notes that premature infants cannot adequately metabolise benzyl alcohol’s metabolite, benzoic acid [8]. The US bacteriostatic water label states “NOT FOR USE IN NEONATES” [1].
- Known sensitivity to benzyl alcohol. Rare, but a reason to use an unpreserved diluent and a single-use approach.
- When a manufacturer says so. A licensed product’s summary of product characteristics (SmPC) overrides any general guidance. Some products come with their own preserved solvent, typically metacresol, and those should be used as supplied.
When bacteriostatic water is the right choice
- Any vial that will be entered more than once, including virtually all research peptides dosed daily or weekly from a single 2, 5 or 10 mg vial.
- Licensed multi-dose products whose labels specify it. The US Gonal-f multi-dose vial, for example, is reconstituted with “Bacteriostatic Water for Injection (0.9% benzyl alcohol), USP” and discarded after 28 days [9].
- Splitting a reconstituted batch into sterile empty vials for later use within the 28-day window.

Does the preservative affect the peptide?
At 0.9%, benzyl alcohol is used in licensed injectable products, including preserved hCG and follitropin diluents, without a stability problem [9][10]. It is not a universal solvent, however. Some peptides are poorly soluble at neutral pH whichever water is used, and suppliers may recommend a small volume of dilute acetic acid or a buffer. A solution that turns cloudy or develops particles should not be used, whichever diluent produced it.

Frequently asked questions
Can I use sterile water instead of bacteriostatic water for peptides?
Only if the whole vial will be used immediately. Sterile water for injection contains no preservative and is labelled for single use, so a vial reconstituted with it and then drawn from over days or weeks has no protection against bacteria introduced by each needle puncture. For any multi-dose vial, a preserved diluent is the appropriate choice.
Is bacteriostatic water the same as saline?
No. Bacteriostatic sodium chloride 0.9% exists and is also preserved with benzyl alcohol, but it is isotonic saline, whereas bacteriostatic water is hypotonic water with no salt. Both are used as diluents. Neither should be injected on its own, and for reconstituting a lyophilised vial the two behave very similarly.
What does bacteriostatic actually mean?
Bacteriostatic means inhibiting bacterial growth rather than killing bacteria outright, which would be bactericidal. The benzyl alcohol in bac water stops organisms introduced during withdrawal from multiplying to dangerous numbers within the 28-day window. It does not sterilise a heavily contaminated vial and it does not replace aseptic technique.
Why can’t sterile water be injected on its own?
Because it is hypotonic. With no dissolved salts, water entering the bloodstream draws into red blood cells by osmosis and can rupture them, which is haemolysis. The label states it must be made approximately isotonic by adding a solute before injection. The same warning applies to bacteriostatic water.
Is bacteriostatic water safe for children?
Benzyl alcohol-preserved products are contraindicated in neonates because of the gasping syndrome. The EMA advises that products containing benzyl alcohol should not be used for more than a week in children under three without medical advice. For any child, the choice of diluent should follow the prescriber’s instructions and the product SmPC, not a general rule.
