Knowing how to store bacteriostatic water is mostly a matter of two dates: the manufacturer’s expiry printed on the label, and the date you first put a needle through the stopper. The first governs the unopened vial. The second starts a 28-day clock that applies no matter how much liquid is left. Everything else, temperature, light, how the vial is handled between uses, exists to make those two dates hold.
This article sets out what the label and the compounding standards actually say, why 28 days and not longer, how storage differs for the diluent itself versus a peptide vial reconstituted with it, and how to tell when a vial should be thrown away. It is written for the UK, where the product is sold in 30 mL multi-dose vials.
How to store bacteriostatic water before opening
The US prescribing information for Bacteriostatic Water for Injection, USP, specifies storage at 20 to 25°C (68 to 77°F), which is controlled room temperature [1]. It does not need refrigerating before opening, and it should not be frozen. Keep it in the carton or otherwise out of direct sunlight, and away from radiators, windowsills and car interiors where temperatures regularly exceed 25°C.
The product is sterile and preserved with 0.9% benzyl alcohol. Benzyl alcohol is chemically stable in aqueous solution over the product’s shelf life, which is why the manufacturer can assign an expiry date of several years to the sealed vial. Does bacteriostatic water expire?
Yes. Every vial carries a manufacturer expiry date, and that date is the outer limit regardless of anything else. The US Centers for Disease Control and Prevention (CDC) puts it plainly for all multi-dose vials: the beyond-use date “should never exceed the manufacturer’s original expiration date” [2]. An unopened vial past its expiry should be discarded even though it may look identical to a fresh one; sterility assurance and preservative potency are only validated to that date.
Before use, inspect the vial. The solution should be clear and colourless with no particles. A cracked vial, a stopper that has been displaced, a missing flip-off cap on a vial you have not opened yourself, or any cloudiness, discolouration or floating material means the vial should not be used, whatever the date says.
Bacteriostatic water after opening: where the 28-day rule comes from
Once the stopper has been punctured, the vial is an opened multiple-dose container, and two authorities set the same limit. USP General Chapter <797>, the United States Pharmacopeia’s standard for sterile compounding, states that a multiple-dose container “may be assigned a BUD of 28 days unless otherwise specified by manufacturer labeling”, regardless of the air quality in which it was opened [3]. The CDC’s injection-safety guidance says that once a multi-dose vial is opened it “should be dated and discarded within 28 days unless the manufacturer states another date for that opened vial” [2].
The 28-day figure rests on antimicrobial effectiveness testing. USP <51> is the test that a preserved product must pass to be labelled multi-dose: the product is deliberately inoculated with defined bacteria and fungi and the preservative must reduce and then hold those populations over a 28-day observation period. It is the compendial preservative-effectiveness test for sterile multi-dose parenterals in the US, European and Japanese pharmacopoeias, and the 28-day multi-dose limit mirrors its observation period [4]. In other words, the preservative has been shown to cope with a challenge over 28 days, and nobody has shown it copes indefinitely.
Three things follow. First, the clock starts at the first puncture, not when the vial is half empty. Second, the limit is independent of refrigeration; cooling an opened vial of bacteriostatic water does not extend it. Third, the rule assumes aseptic technique at every entry. A vial that has been entered with a used needle, or whose stopper was not disinfected, has had its preservative challenged in a way the testing did not anticipate.
Storing a reconstituted peptide vial
A peptide reconstituted with bacteriostatic water is a different object from the diluent itself. It carries the same 28-day multi-dose convention, because the preservative it inherited is what supports repeated entry, but it also contains a dissolved peptide whose chemical stability is usually the shorter-lived of the two concerns. For that reason a reconstituted vial is refrigerated at 2–8°C, protected from light and not frozen, even though the unopened diluent is kept at room temperature.
Manufacturer stability data, where it exists, overrides the general rule. Genotropin (somatropin), for example, is reconstituted with a metacresol-preserved solvent and the summary of product characteristics (SmPC) allows 28 days at 2–8°C [5]. The US Gonal-f multi-dose vial, reconstituted with bacteriostatic water, is discarded after 28 days whether refrigerated or kept at room temperature [6]. The US Pregnyl (hCG) label, with a benzyl alcohol-preserved diluent, states the reconstituted solution is stable for 60 days refrigerated [7]. Practical checklist for an opened vial
- Write the date of first puncture on the vial the moment you open it, and the discard date 28 days later.
- Swab the stopper with 70% isopropyl alcohol and let it dry before every entry. Use a new sterile needle and syringe each time.
- Keep the diluent at room temperature out of light; keep any reconstituted vial in the fridge, upright, in its carton or a small box.
- Do not freeze either. Freezing does not harm the diluent’s benzyl alcohol, but it can cause a reconstituted peptide to aggregate on thawing, and freezing can crack a glass vial.
- Discard the diluent at 28 days after opening or at its expiry date, whichever comes first, even if most of the 30 mL remains.
How much of a 30 mL vial will you actually use?
A typical reconstitution takes 1 to 3 mL. Over a 28-day window that means a single 30 mL vial comfortably serves ten or more reconstitutions, or a whole month of use for several vials at once. Larger diluent vials are not more economical if most of the contents will be discarded at day 28; buy on the basis of what will be used in four weeks. When to discard a vial immediately
- Any visible cloudiness, particles, colour change or crystals.
- A stopper that was entered with a non-sterile needle, or a vial left uncapped with the stopper exposed.
- Suspected contamination of the stopper surface, for example a vial that has been dropped stopper-down.
- A vial that has been through a freeze-thaw cycle or left in a hot environment for an extended period.
- The 28-day mark after opening, or the manufacturer’s expiry date, whichever is earlier.

Frequently asked questions
Does bacteriostatic water need to be refrigerated?
No. The label specifies storage at 20 to 25°C, and refrigeration is not required before or after opening. It does no harm to refrigerate an opened vial, but it does not extend the 28-day limit. A reconstituted peptide vial is a different matter and should be kept at 2–8°C to slow degradation of the dissolved peptide.
How long is bacteriostatic water good for after opening?
Twenty-eight days from the first puncture, or until the printed expiry date if that comes sooner. This is the beyond-use date assigned to opened multi-dose containers by USP <797> and by CDC injection-safety guidance, and it applies regardless of how much liquid remains in the vial.
Can I use bacteriostatic water after 28 days if it still looks clear?
The 28-day limit is not based on appearance. Bacterial contamination is not visible at the levels that matter, and the preservative has only been validated to hold a microbial challenge for 28 days. A clear vial on day 35 may well be fine, but there is no evidence to say so, and the cost of a fresh vial is small.
What happens if bacteriostatic water freezes?
The benzyl alcohol and water are not chemically damaged by freezing, but the glass vial can crack and the stopper seal can be compromised, so a vial that has frozen should be discarded. A reconstituted peptide that has frozen is a bigger problem, because freeze-thaw cycles can cause aggregation and loss of activity.
Does bacteriostatic water expire if unopened?
Yes. The manufacturer’s expiry date applies to sealed vials and should not be exceeded. Manufacturers typically validate sterility and preservative effectiveness only up to that date. Check the label before use and discard any vial that is past it, cracked, cloudy or missing its cap.
